The Ellis Trust for Girls is committed to providing funding for high-impact programming for its grantees. To that end, we require all Ellis Enrichment Grant Program Providers to annually share information about their programming, including their mission and how they provide high quality programming and safe spaces for our Ellis Scholars.

Program Provider Requirements

To ensure that the Trust is meeting its goals and objectives and that the young women who receive funding from the Trust are engaging in programming consistent with the goals of the Ellis Trust, organizations which will receive payments from the Trust (“Program Providers”) for programs are required to do the following annually.

  1. Complete and return the attached Parameters for High-Impact Programming Checklist for each program for which you will receive payment. The Checklist includes a Certification regarding child abuse clearances and criminal record checks for the Program Provider’s employees who will have direct contact with program participants. This Checklist will help inform approved programming in the future.
  2. Provide an ACORD Certificate of Liability Insurance evidencing insurance in the following types and amounts:
    • The following minimum Commercial General Liability limits and terms must be provided by an insurance carrier with an A.M. Best rating of A-VII or better:
      • $1,000,000 Each Occurrence
      • $1,000,000 Personal Injury & Advertising Injury
      • $2,000,000 Products/Completed Operations Aggregate
      • $2,000,000 General Aggregate
    • Heights Philadelphia and the Ellis Trust for Girls must be named as Additional Insureds under your Commercial General Liability Policy.
    • For programs that provide transportation, minimum Automobile Liability of $1,000,000 per accident including coverage for owned, non-owned and hired vehicles.
    • Workers’ Compensation as required by statute and Employers Liability limits of $500,000 per accident/ $500,000 disease-policy limit/ $500,000 disease-each employee.
  3. Provide a completed IRS W-9 for your organization with your taxpayer ID.
  4. After being designated as an approved program provider complete a Program Provider Report Card following the conclusion of each program attended by an Ellis participant.  The Program Provider Report Card will be an online survey. The link to the survey will be sent to you when a payment is made to your organization by the Ellis Trust.

Apply to Become an Ellis Trust Program Provider

Program Providers can apply to be considered as approved Ellis Trust program providers at any time by providing the documentation requested above. The completed Parameters for High-Impact Programming Checklist, ACORD form and IRS W-9 should be scanned and emailed to ellis@heights.org.  Please be aware, the Ellis Trust will not be able to fund programs offered by Program Providers that have not complied with these annual program eligibility requirements.
